THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

We are dedicated to safeguarding the privacy of your protected health information (PHI), using and disclosing it only as necessary to provide you with healthcare products and services.

How We Use and Disclose Your PHI

We primarily use and disclose your PHI in relation to the healthcare products and services we provide you, such as dispensing your prescriptions. Specifically, we use and disclose your PHI as necessary for treatment, obtaining payment, and other healthcare operations. We work closely with our US-licensed prescribers and dispensing pharmacy partners to ensure that your safety and privacy are always prioritized in our services.

Your Rights with Respect to Your PHI

HIPAA provides you with several rights related to your PHI:

- You have the right to inspect and receive this written Notice of Privacy Practices and a photocopy of our records containing your PHI at any time, with reasonable fees for the copies.
- You can request limitations on our use and disclosure of your PHI, though we may not be able to agree if it affects our ability to provide you with healthcare products and services or if required by law.
- You can request changes in the content of your PHI if you believe it is incomplete or inaccurate, with explanations if we cannot comply.
- You can request for confidential communications and specify locations or means for communication.
- You can obtain a written record of some disclosures of your PHI, with one record provided per twelve (12) -month period at no charge.

Ways That We May Use and Disclose Your PHI

- Treatment: We use your PHI to provide and coordinate healthcare services.
- Payment: We use your PHI for billing and reimbursement purposes.
- Healthcare Operations: We use your PHI for activities related to providing healthcare products and services.
- Business Associates: We may disclose your PHI to business associates who help us provide healthcare products and services.
- Disclosures Not Involving Treatment, Payment, and Healthcare Operations: We may disclose your PHI for purposes such as healthcare communications, patient profile printouts, and disclosures required by law.
- Federal and State Government Agencies: We may disclose your PHI to government agencies for health monitoring and compliance purposes.
- Legal Disputes: We may disclose your PHI in response to court orders, subpoenas, or legal proceedings.
- Public Health and Safety: We may disclose your PHI to protect public health or safety.
- National Security and Intelligence: We may disclose your PHI for national security or intelligence purposes.
- Military and Veterans: We may disclose your PHI for military or veterans' health programs, if applicable
- Correctional Institution: If you are an inmate, we may disclose your PHI to the institution, or its officers, when required
- Research: We may disclose your PHI to conduct any stage of a research and to disclose to researchers, as authorized by law.
- Miscellaneous Disclosures: We may make other disclosures, without your written authorization, as allowed by HIPAA and as required by law.

Your Rights Not Contained in this Notice

If a use or disclosure of your PHI is not described in this Notice, we will obtain your written authorization before the use or disclosure. You have the right to refuse or revoke such authorizations.

Changes in this Notice

AlwaysPharma adheres to the terms of this Notice and any change that may take effect. We reserve the right to change our practices and this Notice at any time. All changes will be reflected in the updated Notice which will be posted on our website with the new effective date. Upon request, you may also be provided with the revised Notice.

Our State-Specific Laws

In addition to HIPAA, our state may enact laws providing you with additional privacy and other rights related to your health care and PHI.

HIPAA requires that we give you this Notice and make a good faith effort to obtain your written acknowledgment. You will be asked to sign a document acknowledging that you received this Notice.
